 A Home Care Package provider receives and manages subsidies for aged care services on your behalf. Their level of responsibility for delivering your services and managing your fees and payments will depend on your choice of provider. 

Trilogy Care manages the funding and invoice payments and oversees the budget, linking with My Aged Care.

Safe Solutions Care provides Care Coordination. We don't employ carers within our services.

We liaise with other providers to source the best fit for you, you can choose who supports you, and change at any time.

The Home Care Package program has been designed to:

• To provide personalised care and support based on individual needs and preferences.

Services provided under a Home Care Package is subject to your personal care plan but can include: 

• Personal care: Assistance with bathing, dressing, grooming, and mobility.
• Nursing care: Medical support, medication management, and wound care.
• Domestic assistance: Help with cleaning, laundry, and meal preparation.
• Social support: Companionship, social activities, and community engagement.
• Transportation: Assistance with shopping, medical appointments, and social outings.
• Home modifications: Installing safety aids, such as handrails and ramps, to improve home accessibility.

We offer you care coordination support so you can maximise your Home Care Package budget and minimise your stress, so you can continue to live life your way, at home.

Working with a Trilogy Care care coordinator to manage your Home Care Package gives you the freedom to:

• Design your care plan and budget with your dedicated Trilogy Care care partner
• Gain more control over how your Home Care Package funds are spent
• Flexible care coordination support that allows you to select your care workers and support services
• Increase your care hours with lower hourly rates and package management fees

Here at Safe Solutions Care we want to help you stretch you home care package funding, and get all the help you can from your budget.

From 1 November, when the new changes take effect, our fee structure will be aligned with the My Aged Care changes.

My Aged Care allows Care Coordinators to charge a  0 - 30%  fee of the total budget spent in each category.

 Safe Solutions Care has calculated the fees to ensure we cover our costs and support you, while maximizing your funds to enable the most support possible.

Safe Solutions Care fees are set at 20% of the funds spent.

This is a little less than our fees under the existing system, which is 11% of the total budget.

Under the Support at Home program, participants will see three major changes:

• Quarterly budgets will replace daily subsidies, providing more predictability and flexibility when planning care
• Services will be grouped into three structured categories: Clinical Care, Independence Support and Everyday Living Support
• New participant classifications—Grandfathered, Hybrid, and New—will determine funding rules and fee contributions

These changes aim to simplify the care experience while aligning services with each person’s needs and goals.
